CASE OF THE WEEK – “A case of Bilateral Avascular Necrosis Femoral Head” by Dr. Pawan Nath Specialist, Orthopaedic Surgeon, NMC Royal Hospital Sharjah

A case of Bilateral Avascular Necrosis Femoral Head, the patient visited us with c/o pain both hip more on left side since more than 1 year. Since 1 year patient was visiting multiple places for his knee pain which was actually a referred pain having pathology in his hip.

Patient had restriction of flexion in both hip but more on the left hip. He had pain while walking and it’s slowly started affecting his day today activity like walking long distance, driving and others.

MRI of both hip showed grade 4 and grade 3 avascular necrosis of femoral head in left and right hip respectively.

He had the Harris hip score evaluated which was less than 70 at the later stages and finally the total hip replacement was planned.

I have used uncemented total hip replacement on both sides. Since the patient was 41 years old with good bone strength, I was able to use the acetabular cup without screw fixation.

Presently post op 1 year 3 months, patient is doing excellent with good range of movement of both hip with no pain. The patient can walk longer, drive comfortably and do all his routine activity pain free.
