CASE OF THE WEEK – “Myocardial Infarction in young” by Dr. Mohammed Shabbir, Specialist, Emergency Medicine and Dr. Adel Eryani, Consultant, Interventional Cardiology at NMC Royal Hospital Sharjah

Introduction

Although myocardial infarction (MI) mainly occurs in patients older than 45, young men or women can suffer MI. Fortunately, its incidence is not common in patients younger than 45 years which is proving to be not true especially in patients of south asian countries. The disease carries a significant morbidity, psychological effects, and financial constraints for the person and the family when it occurs at a young age. The causes of MI among patients aged less than 45 can be divided into four groups: (1) atheromatous coronary artery disease; (2) non-atheromatous coronary artery disease; (2) hyper-coagulable states; (4) MI related to substance misuse. There is a considerable overlap between all the groups.

Case Report

This 38 year old gentleman of south Asian decent presented to ER at around 10:50 AM with the history of chest pain from last 3 hrs radiating to back.

Patient was evaluated and found to have Acute Anteroseptal ST elevation MI.

Cathlab activated and cardiologist on duty alerted, the patient shifted to cathlab is lessthan 10 minutes of arrival. In coronary angio he was found to have double vessel disease and Done Primary PCI to ostial-proximal LAD and PCI to mid LAD and LCX.,

Conclusion

Even-though Myocardial infarction is the disease of middle age but recent trends are proving it wrong especially in patient’s of South Asian descents. Early recognition and treatment reduces morbidity and mortality in patients and there by increases productivity in and quality of life in patients
